Hall of Fame Nomination Form
Thank you for your interest in completing a nomination to be considered for the St. Joseph’s Catholic School Athletic Hall of Fame. Please complete all sections of the form to the best of your ability. All nominations are to be received on or before February 22, 2023 for consideration in the 2023 induction ceremony. All information submitted shall be retained by the St. Joseph’s Catholic School Athletic Hall of Fame Selection Committee. Nominators can upload supporting materials (newspaper clippings, magazine articles, yearbook photos, etc.) on the form. It is up to the nominator to prove that the candidate meets the requirements of the Athletic Hall of Fame. It is very important to include as thorough a history as possible, justifying in detail why your nominee is deserving of membership in the SJCS Athletic Hall of Fame.
